programs in recent years. <i>But how do you know they actually work?</i><o:p></o:p></span></div>

central question addressed in an <a href="http://www.jmir.org/2017/3/e90/">original
paper</a> by Mary AM Rogers, MS, PhD, and colleagues at the University of
Michigan. The results of their study are published in a recent issue of
the <i>Journal of Medical Internet Research.</i> (Please see full

comprehensive search, review and analysis of studies that tested the
effectiveness of programs in randomized clinical trials (the gold standard in
medical research), the Dr. Rogers and colleagues found a wide range of programs
for health-related behaviors and disease prevention and support. However,
the majority of programs were only available to clinical trial participants.<o:p></o:p></span></div>

trials is based on a group effect and the program will not necessarily benefit
every single person. They note that programs "do not guarantee a specific
result; they only promise a greater likelihood of a benefit if the therapy is
completed."<o:p></o:p></span></div>

When Cecilia Conrath and Leonard Doak married in 1973, she was a commissioned officer in the U.S. Public Health Service, developing and leading continuing education for physicians and allied health professionals. He was an engineer who volunteered as a literacy tutor for adults.</span></span><br />

<span style="font-size: small;"><span style="font-family: "trebuchet ms" , sans-serif;">They described, in a <a href="http://www.blogger.com/goog_147545592">2009 Literacy Out Loud Podcast</a> with Helen Osborne, how a conversation early in their marriage about the difficulty Len's students had understanding medical advice led to their groundbreaking work. Over the past week, the health literacy community remembered Len with fondness and respect. <span style="font-size: small;">Patients, too, need to be educated about their role and given the tools and emotional support they need to fully participate in the process—without fear of criticism or judgment. Dr. Barry and Ms. Edgman-Levitan believe that by adopting the principles of shared decision making, "clinicians can help patients understand the importance of their values and preferences in making the decisions that are best for them."</span><br />
<br />
This is especially relevant when a "patient arrives at a crossroads of [acceptable] medical options" each of which is associated with benefits and risks that may have profound and lasting effects. The authors cite as good examples: "... therapy for early stage breast cancer or prostate cancer, lipid-lowering medication for the primary prevention of coronary heart disease, and genetic and cancer screening tests...."</div>

performance measures (52 percent), and inadequate time spent with patients (40
percent). Sixty two percent said diagnostic testing would be reduced if not for
financial incentives to specialists (39 percent for primary care physicians).</span><br />

Foundation's <a href="http://www.rwjf.org/pioneer/">Pioneer Portfolio</a> and the <a href="http://www.seriousgames.org/">Serious Games Initiative</a>, Games for Health has built an international community that includes 10,000 of the best minds in game development, research, and medicine. Their goal: to use cutting-edge games and game technologies to improve health and the delivery of health care.<br />
<br />
No longer confined to family rooms, games are being used in hospitals, gyms, and schools. They are empowering people with physical <a href="http://www.blogger.com/goog_1430679128">(</a><a href="http://goog_1430679128/">NIH Grant, Parkinson's Games)</a> and cognitive disabilities <a href="http://www.lumosity.com/personal-training-plan">(Lumosity)</a>.<br />
<br />
<a href="https://www.superbetter.com/">SuperBetter</a> is a game that helps people recover from an illness and/or achieve a health goal. The game was created by <a href="http://janemcgonigal.com/">Jane McGonigal</a>, Creative Director of Social Chocolate, as she found herself struggling to recover from a severe concussion. McGonigal says that within a few weeks, she was feeling better and recovering faster than she'd thought possible. Based on her experience, she believed others could benefit, too.<br />
<br />
Ben Sawyer's company, <a href="http://www.dmill.com/">Digitalmill</a>, is working with Yale School of Medicine to develop Play2Prevent™, a game to help youth at risk of HIV. Games are also being used to distract children who undergo frequent and painful medical procedures as well as to train health care providers in clinical skills. <a href="http://www.breakawaygames.com/serious-games/solutions/healthcare/">Free Dive and Pulse!! </a> by Breakaway Games are a few examples. </div>

Many others, like <a href="http://www.playnormous.com/">Playnormous</a>—which was developed with researchers at Baylor College of Medicine, the University of Texas Health Science Center, US Department of Agriculture and National Institutes of Health—are designed to teach children about health with an emphasis on exercise and nutrition.<br />

Two of the nation's largest health benefits company Humana (<a href="http://www.humanagames.com/">Farmscape and the Horsepower Challenge)</a> and Aetna <a href="http://www.mindbloom.com/about/">(Mindbloom)</a> have also developed games as have a number of pharmaceutical companies. <br />

One game that has been getting a lot of attention is <a href="http://about.zamzee.com/">Zamzee</a> which was developed to encourage physical activity in young teens. Children wear a smart device to record how much they move. The more they move the more rewards they earn to go shopping, take on challenges, and compete with friends online.<br />

<b>Health Games </b><b>Research </b><br />

In <a href="http://www.ncbi.nlm.nih.gov/pmc/articles/PMC2189579/pdf/nihms36392.pdf"><i>Playing for Real: Video Games and Stories for Health Related Behavior Change</i></a>, a review of the literature on video games through 2006, Baranowski and colleagues found that most of the articles showed that games promoted health-related behavior change. The authors concluded that "research is needed on the optimal use of game-based stories, fantasy, interactivity, and behavior change technology in promoting health-related behavior change."<br />
<br />
In recent years, the field has been moving steadily in that direction. Funded by Robert Wood Johnson Foundation's Pioneer Portfolio, Health Games Research is a national program that provides scientific leadership and resources in the field (see Additional Resources below).<br />

<span style="font-size: small;">As Dr. Miller tells it, the story of Motivational Interviewing
(MI) is one of serendipity. It began in 1973 when he was in Milwaukee on an
internship and "wandered into an alcohol treatment center, knowing
absolutely nothing about alcoholism." "Having nothing to offer," he
put on "his Carl Rogers hat" and spent the summer listening to the men on the ward. He listened to "their
stories of how they had gotten there, where they thought their lives
were going and what their hopes and dreams were."</span></div>

<span style="font-size: small;">Feeling
an "immediate chemistry working with people with alcohol problems," Dr.
Miller decided to do his dissertation on problem drinking. After receiving his Ph.D. in clinical psychology from the University of Orgeon, he joined the faculty at the University of New Mexico in 1976. </span><br />
<br />
<span style="font-size: small;">Dr. Miller's continued research on alcoholism eventually led him to the conclusion that a person's innate capacity for change was enhanced by counselors who practiced reflective listening and had strong empathic skills. Clients who were in treatment with empathic counselors had significantly better success in stopping drinking than those whose counselors were less empathetic.</span></div>

In 1991while on sabbatical in Norway, the questions of
young psychologists that arose during role play of clinical scenarios "forced" Dr. Miller to verbalize the core principles he used intuitively in practice and prompted him to write down a "conceptual model and some clinical guidelines":<br />

<li> Counselor's role is to </li>

<li>Minimize resistance without confrontation (which was the standard in addiction counseling at the time)</li>

<b>Fortuitous Meeting with Dr. Stephen Rollnick</b><br />
<br />
Several years later, on a second sabbatical in Australia, Dr. Miller met Stephen
Rollnick, Ph.D. who told him MI was being used widely for addiction treatment in the UK and encouraged him to publish more about it. This encounter resulted in their coauthoring the first book on MI.<br />
<br />
Their collaboration led to further refinements of MI and Dr. Miller credits Dr. Rollnick with the addition of exploring and resolving a client's ambivalence about change as a key concept.</div>

of Primary Care & Public Health, School of Medicine, Cardiff
University, Wales, UK—has pioneered new applications of MI in healthcare. Like Dr. Miller, he has written a number of books with colleagues on MI and has been published widely in scientific journals. Dr. Rollnick is also one of the cofounders of the MINT (Motivational Interviewing Network of Trainers) and runs workshops in diverse cultures all around the world.<br />

<b>MI is Evidence-Based</b><br />

<span style="font-size: small;">Motivational Interviewing is "a collaborative person-centered
form of guiding to elicit and strengthen motivation for change." Originally used in substance abuse disorders, MI has be found effective
in reducing problem behaviors as well as in promoting healthy behaviors
and adherence to treatment in chronic illness. </span><br />

<span style="font-size: small;">MI has been thoroughly evaluated and widely disseminated over the past thirty years:</span></div>

<li><span style="font-size: small;">More than 200 randomized trials, including 10 multicenter trials, have been conducted

<span style="font-size: small;">Dr. Miller cautions that MI cannot be learned by simply "reading a book, watching a video, or attending a workshop." Counselors undergo intensive training that is centered on both the "spirit of MI" as well as skills and strategies needed to bring that spirit to life. </span><br />

<span style="font-size: small;">Relationship
matters. A person's resolution of ambivalence and movement to change is
positively influenced by someone "who is kind to them, who honors them,
and offers hope" at a time when they may not have any. Dr. Miller
believes there are "two things at work: the spirit of MI and helping
clients verbalize and move forward when ready."</span><br />
<br />
<span style="font-size: small;">With its core principles of empathy, reflective listening, and respect for autonomy, MI crosses cultures very well and requires little adaption. It is brief; specifiable; generalizable to many conditions (addiction, medical, criminal justice); complementary with other therapies, and can be learned by a broad range of providers.</span></div>

If there is a takeaway for those of us who are not counselors or providers, it is in remembering the power of listening and of empathy in our daily interactions and the uselessness of pushing ourselves or others to change before we or they are ready (<a href="http://www.amazon.com/Changing-Good-Revolutionary-Overcoming-Positively/dp/038072572X">Changing for Good</a> has excellent advice on this topic).<br />

And for those of us who are recovering fixers, there is a quote by British novelist Terry Pratchett on Dr. Rollnick's website that may apply: "After all, when you seek advice from someone it's certainly not because you want them to give it. You just want them to be there while you talk to yourself." <br />

One of the most inspiring talks was given by Dave deBronkart also known as e-patient Dave. Diagnosed with stage IV kidney cancer in 2007 (with a median survival rate of 24 weeks), he used the Internet to learn everything he could about the disease and treatment options so he could partner with his healthcare team. Today he is well.<br />
<br />
In his presentation, Dave poses the question: "How can it be that the most useful and relevant and up-to-the-minute information can exist outside traditional channels?" While he doesn't suggest that everyone <b><i>should</i></b> do as he did or that others will have the same results, Dave certainly stands as a powerful example of someone who is empowered, engaged, equipped and enabled. He ends his presentation with a challenge to physicians: "Let patients help, we might surprise you."<br />

When she was three years old, Kate Lorig was diagnosed with Gaucher disease—a rare hereditary disorder caused by an enzyme deficiency and characterized by enlargement of the spleen and liver, bone lesions, and in some cases neurological impairment. At age nine, her parents were told that she would be "retarded." Instead she had a normal childhood and went on to make remarkable contributions to the field of patient education. </div>

Dr. Lorig, who has a master's degree in nursing and a doctorate in public health, is Professor of Medicine and Director of the Patient Education Research Center at Stanford University. She began her career at Stanford's Arthritis Center as a research associate in 1978 and, with the help of arthritis specialists, health educators, physical and occupational therapists, she developed the first Arthritis Self-Management Program.</div>

During the six-week program, small groups of patients meet to talk about the day-to-day challenges of living with a chronic illness with an emphasis on:<br />

<li>Problem-solving and patients helping patients</li>
<li>Managing pain, fatigue, depression </li>
<li>Exercise, healthy eating, relaxation techniques</li>
<li>Communication skills</li>
<li>Goal setting and action planning </li>

<li>Self-tailoring (patients using tools that work best for them)</li>
</ul>

The program is offered throughout the U.S. by the Arthritis Foundation and has also been offered in Canada, the United Kingdom, Australia, New Zealand, South Africa, China, Scandinavia, and St. Lucia. It has become the prototype for all the Stanford programs which cover a range of chronic conditions and are offered in community settings as well as on the Internet. <br />

Proven effective in randomized clinical trials, the programs are designed to help participants "gain self-confidence in their ability to control their symptoms and how their health problems effect their lives."<br />
